Hayward: "Blow out preventers…across the world…not fail safe"

Hayward tells Rep. Barton that he thinks one of the biggest industry lessons going forward will be to make sure the blow out preventers on these wells are actually "fail safe". Right now, he admits, blow out preventers around the world, not just for BP, are not "fail safe".
But when Stupak proceeds to cite an internal BP document claiming that there were 260 failure modes that might require pulling of the blow out preventer on Deepwater Horizon, Hayward responds yes to whether blow out preventers are suitable fail-safe mechanisms.
Hayward has an inexplicable amount of faith, contrary to evidence, in the potential of a blowout preventer to prevent catastrophes like Deepwater Horizon in the future–especially considering he has admitted himself that status quo blowout preventers are not fail-safe.
